Work out cleaner-hours per visit at your loaded rate, add supplies, add overhead, and that is your break-even per visit. Multiply by visits per month (a month is 52/12 = 4.33 weeks, not 4) before you quote a monthly figure.
Per-square-foot rates are frequency-blind - the same building cleaned daily and weekly are completely different jobs at the same square footage. Price from production rate (square feet per cleaner-hour) and check the result against the band.
